Vy\u0161etrenie vykon\u00e1vame nielen u&nbsp;onkologicky chor\u00fdch pacientov Prot\u00f3nov\u00e9ho centra, ale aj u&nbsp;v\u0161etk\u00fdch ostatn\u00fdch pacientov, ktor\u00fdm o\u0161etruj\u00faci lek\u00e1r vystav\u00ed \u017eiadanku.<\/p>\n<h3>Oddelenie Nukle\u00e1rnej medic\u00edny vykon\u00e1va nasleduj\u00face vy\u0161etrenia:<\/h3>\n<ul>\n<li>PET\/CT trupu s&nbsp;vyu\u017eit\u00edm r\u00e1diofarmaka FDG (fluorodeoxygluk\u00f3za),<\/li>\n<li>PET\/CT trupu s&nbsp;vyu\u017eit\u00edm r\u00e1diofarmaka FCh (fluorochol\u00edn),<\/li>\n<li>PET\/CT trupu s&nbsp;vyu\u017eit\u00edm r\u00e1dioafarmaka FCV (fluciclovinu\/Axum\u00edn),<\/li>\n<li>PET\/CT mozgu s&nbsp;vyu\u017eit\u00edm r\u00e1diofarmaka FMISO (fluoromisonidazole),<\/li>\n<li>PET\/CT prsn\u00edka s&nbsp;pou\u017eit\u00edm r\u00e1diofarmaka FES (fluorestradiol)<\/li>\n<li>PET\/CT pre pl\u00e1novanie r\u00e1dioterapie.<\/li>\n<\/ul>\n<p>Term\u00edny objednania s\u00fa kr\u00e1tke, jedn\u00e1 sa o&nbsp;rozmedzie 3 &#8211; 4 t\u00fd\u017ed\u0148ov. Ako objedna\u0165 pacienta na&nbsp;PET\/CT vy\u0161etrenie<\/h3>\n<p>Na&nbsp;vy\u0161etrenie PET\/CT, teda vy\u0161etrenie pozitr\u00f3novou emisnou tomografiou v&nbsp;kombin\u00e1cii s&nbsp;po\u010d\u00edta\u010dovou tomografiou, objedn\u00e1te pacientov na&nbsp;telef\u00f3nnom \u010d\u00edsle +420&nbsp;222&nbsp;999&nbsp;046. N\u00e1sledne za\u0161lite vyplnen\u00fa \u017eiadanku na&nbsp;e-mail:zadanky@ptc. Rezervovanie term\u00ednu a&nbsp;zaslanie \u017eiadanky vopred zjednodu\u0161\u00ed a&nbsp;zr\u00fdchli syst\u00e9m objednania.<\/p>\n<h3>2. PET\/CT &#8211; na&nbsp;stiahnutie pre lek\u00e1rov tu:<\/h3>\n<p><a href=\"https:\/\/www.ptc.cz\/wp-content\/uploads\/2024\/09\/Brozura_A5_PET_CT_aktualizovano_2023_nahled.pdf\">Pou\u010denie pre pacienta PET\/CT vy\u0161etrenie<\/a> (PDF na&nbsp;stiahnutie)<\/p>\n<p>Aby sme v\u00e1\u0161ho pacienta u\u0161etrili pr\u00edpadn\u00fdch ob\u00e1v z&nbsp;vy\u0161etrenia, pripravili sme po \u0148om z\u00e1kladn\u00e9 inform\u00e1cie o&nbsp;PET\/CT vy\u0161etren\u00ed.<\/p>\n<p><a href=\"https:\/\/www.ptc.cz\/wp-content\/uploads\/2024\/09\/Informovany-souhlas-PETCT-CS_verze-8.docx\">Informovan\u00fd s\u00fahlas pacienta s&nbsp;PET\/CT vy\u0161etren\u00edm<\/a> (na&nbsp;stiahnutie)<\/p>\n<p>Informovan\u00fd s\u00fahlas s&nbsp;PET\/CT vy\u0161etren\u00edm si m\u00f4\u017ee pacient pre\u0161tudova\u0165 v&nbsp;pokoji domova. M\u00f4\u017ee ho vyplni\u0165 a&nbsp;podp\u00edsa\u0165 a\u017e pri n\u00e1v\u0161teve Protonov\u00e9ho centra.<\/p>\n<h3>3.&nbsp;\u010co je PET\/CT<\/h3>\n<p>PET\/CT je diagnostick\u00e1 zobrazovacia met\u00f3da nukle\u00e1rnej medic\u00edny. Met\u00f3da sa pou\u017e\u00edva najm\u00e4 na&nbsp;diagnostiku n\u00e1dorov\u00fdch ochoren\u00ed, pr\u00edpadne z\u00e1palov, pri\u010dom vyu\u017e\u00edva f\u00faziu alebo kombin\u00e1ciu funk\u010dn\u00e9ho (metabolick\u00e9ho) a&nbsp;anatomick\u00e9ho (morfologick\u00e9ho) obrazu.<\/p>\n<p>PET\/CT vy\u0161etrenie patr\u00ed v&nbsp;r\u00e1mci zobrazovac\u00edch met\u00f3d k&nbsp;najmodernej\u0161\u00edm. Kombinuje v\u00fdhody metabolick\u00e9ho vy\u0161etrenia (PET) a&nbsp;\u0161truktur\u00e1lneho zobrazenia (CT). Ide o&nbsp;tzv. hybridn\u00e9 zobrazenie. V\u010faka tomu umo\u017e\u0148uje ur\u010di\u0165 presn\u00fa anatomick\u00fa lokaliz\u00e1ciu patologick\u00e9ho n\u00e1lezu a&nbsp;z\u00e1rove\u0148 objasni\u0165 nejasn\u00e9 \u0161truktur\u00e1lne zmeny. T\u00fdm sa v\u00fdznamne zvy\u0161uje v\u00fd\u0165a\u017enos\u0165 oboch met\u00f3d. Vykonanie oboch vy\u0161etren\u00ed s\u00fa\u010dasne zjednodu\u0161uje mnohokr\u00e1t zd\u013ahav\u00fd, pre pacientov n\u00e1ro\u010dn\u00fd diagnostick\u00fd proces.<\/p>\n<ul>\n<li>Naj\u010dastej\u0161ie vyu\u017e\u00edvan\u00e1 met\u00f3da v&nbsp;onkol\u00f3gii \u2013 u&nbsp;pacientov s&nbsp;podozren\u00edm na&nbsp;n\u00e1dorov\u00e9 ochorenie alebo s&nbsp;u\u017e potvrden\u00fdm n\u00e1dorov\u00fdm ochoren\u00edm. V&nbsp;tomto pr\u00edpade sa vy\u0161etrenie vykon\u00e1va pred za\u010dat\u00edm lie\u010dby (na&nbsp;z\u00e1klade tohto vy\u0161etrenia doch\u00e1dza k&nbsp;zmene lie\u010debn\u00e9ho postupu a\u017e v&nbsp;25&nbsp;% pr\u00edpadov). Vy\u0161etrenie sa ale tie\u017e \u010dasto vykon\u00e1va ako kontrola po vykonanej lie\u010dbe (zhodnotenie efektu lie\u010dby a&nbsp;pre jej pr\u00edpadn\u00fa \u00fapravu) alebo pre pl\u00e1novanie r\u00e1dioterapie.<\/li>\n<li>PET\/CT sa taktie\u017e uplat\u0148uje v&nbsp;diagnostike z\u00e1palov\u00fdch, resp. autoimunitn\u00fdch ochoren\u00ed, v&nbsp;neurol\u00f3gii av kardiol\u00f3gii.<\/li>\n<\/ul>\n<p><strong>Na\u0161e pracovisko je vybaven\u00e9 pr\u00edstrojom: PET\/CT Siemens Biograph Vision 450<\/strong><\/p>\n<p>Hybridn\u00fd pr\u00edstroj na&nbsp;vy\u0161etrenie PET\/CT, teda pozitr\u00f3nov\u00fa emisn\u00fa tomografiu v&nbsp;kombin\u00e1cii s&nbsp;v\u00fdpo\u010dtovou tomografiou, kombinuje v\u00fdhody metabolick\u00e9ho vy\u0161etrenia (PET) a&nbsp;\u0161truktur\u00e1lneho zobrazenia (CT). Jeho pomerne kr\u00e1tke a&nbsp;nad\u0161tandardne \u0161irok\u00e9 gantry, \u010di\u017ee \u201etunel\u201c, s&nbsp;priemerom 78 cm a&nbsp;vy\u0161etrovac\u00ed st\u00f4l s&nbsp;nosnos\u0165ou a\u017e 227&nbsp;kg umo\u017e\u0148uj\u00fa nielen vy\u0161etrenie pacientov statnej postavy a&nbsp;pacientov citliv\u00fdch na&nbsp;stiesnen\u00e9 priestory, ale aj vy\u0161etrenie s&nbsp;pom\u00f4ckami pre pl\u00e1novanie r\u00e1dioterapie. V\u010faka vysokej efektivite sn\u00edmania umo\u017e\u0148uje zn\u00ed\u017ei\u0165 aplikovan\u00fa aktivitu r\u00e1diofarmaka, a&nbsp;teda vo v\u00fdsledku aj mo\u017enos\u0165 zn\u00ed\u017eenia radia\u010dnej z\u00e1\u0165a\u017ee pacientov. Pr\u00edstroj je tie\u017e vybaven\u00fd algoritmom na&nbsp;potla\u010denie metalick\u00fdch artefaktov, napr. v&nbsp;okol\u00ed k\u013abov\u00fdch n\u00e1hrad.<\/p>\n<h3>4. Pou\u017eitie r\u00e1diofarm\u00e1k<\/h3>\n<p>Na&nbsp;zhotovenie metabolick\u00e9ho obrazu je potrebn\u00e1 intraven\u00f3zna aplik\u00e1cia \u0161peci\u00e1lnych r\u00e1dioakt\u00edvnych l\u00e1tok, naz\u00fdvan\u00fdch r\u00e1diofarmak\u00e1, (naj\u010dastej\u0161ie F-fluordeoxygluk\u00f3za (FDG)). Aplikovan\u00e9 r\u00e1dioakt\u00edvne l\u00e1tky sa z&nbsp;tela pacienta r\u00fdchlo vylu\u010duj\u00fa mo\u010dom, nep\u00f4sobia alergie ani neobmedzuj\u00fa riadenie motorov\u00fdch vozidiel.<\/p>\n<h4>FDG (fluorodeoxygluk\u00f3za)<\/h4>\n<p>FDG je anal\u00f3g gluk\u00f3zy, ktor\u00fd m\u00e1 tendenciu sa hromadi\u0165 v&nbsp;tkaniv\u00e1ch s&nbsp;vysokou potrebou gluk\u00f3zy, ako s\u00fa n\u00e1dory a&nbsp;z\u00e1palov\u00e9 bunky.<\/p>\n<p><strong>Indik\u00e1ciami na&nbsp;vy\u0161etrenie s\u00fa:<\/strong><\/p>\n<p>Diferenci\u00e1lna diagnostika ben\u00edgnych a&nbsp;mal\u00edgnych lo\u017eiskov\u00fdch l\u00e9zi\u00ed, p\u00e1tranie po nezn\u00e1mom prim\u00e1rnom mal\u00edgnom lo\u017eisku, stanovenie rozsahu onkologick\u00e9ho ochorenia, monitorovanie efektu terapie, diagnostika recid\u00edvy n\u00e1dorov\u00e9ho ochorenia, pl\u00e1novanie rozsahu r\u00e1dioterapie, detekcia z\u00e1palov\u00e9ho lo\u017eiska.<\/p>\n<h4>F-FMISO (fluoromizonidazole)<\/h4>\n<p>18F-FMISO sl\u00fa\u017ei jednak na&nbsp;preukaz isch\u00e9mie v&nbsp;n\u00e1dore, \u010do m\u00e1 v\u00fdznam najm\u00e4 pre sp\u00f4sob lie\u010dby, ale pou\u017e\u00edva sa aj na&nbsp;odhad progn\u00f3zy ochorenia.<\/p>\n<p><strong>Medzi hlavn\u00e9 indik\u00e1cie v&nbsp;s\u00fa\u010dasnej dobe patria:<\/strong><\/p>\n<ul>\n<li>N\u00e1dory mozgu (gli\u00f3my)<\/li>\n<li>N\u00e1dory v&nbsp;oblasti hlavy a&nbsp;krku (najm\u00e4 dla\u017edicobunkov\u00e9 karcin\u00f3my)<\/li>\n<li>Nemalobunkov\u00fd p\u013e\u00facny karcin\u00f3m (NSCLC)<\/li>\n<li>Karcin\u00f3m obli\u010dky<\/li>\n<li>Karcin\u00f3m prsn\u00edka<\/li>\n<li>Sarkom<\/li>\n<\/ul>\n<h4>FCh (fluorochol\u00edn)<\/h4>\n<p><strong>Indik\u00e1ciami na&nbsp;vy\u0161etrenie s\u00fa:<\/strong><\/p>\n<p>Karcin\u00f3m prostaty, lokaliz\u00e1cia aden\u00f3mu \u010di hyperpl\u00e1zia pri\u0161t\u00edtnych teliesok pri podozren\u00ed na&nbsp;prim\u00e1rnu hyperparatyre\u00f3zu.<\/p>\n<h4>FCV (fluciclovine\/Axum\u00edn)<\/h4>\n<p>Indik\u00e1ciami na&nbsp;vy\u0161etrenie s\u00fa: Detekcia recid\u00edvy karcin\u00f3mu prostaty.<\/p>\n<\/div>\n<div class=\"col js-imageOptimalization\">\n<p>Morfologick\u00fd obraz z\u00edskavame pomocou plnohodnotn\u00e9ho CT vy\u0161etrenia s&nbsp;aplik\u00e1ciou j\u00f3dov\u00e9ho kontrastu (pokia\u013e nie je kontraindikovan\u00fd). Vy\u0161etrenie je \u010dasovo n\u00e1ro\u010dn\u00e9, jedn\u00e1 sa pribli\u017ene o&nbsp;3 hodiny, preto\u017ee sa \u0161tandardne sn\u00edma ve\u013ek\u00fd rozsah tela (naz\u00fdvan\u00e9 celotelov\u00e9 vy\u0161etrenie).<\/p>\n<h4><strong>FES (fluoroestradiol)<\/strong><\/h4>\n<p>Vy\u0161etrenie pomocou 18F-FES\/PET je mo\u017en\u00e9 vykona\u0165 pri karcin\u00f3me prsn\u00edka:<\/p>\n<ul>\n<li>Pre staging a&nbsp;restaging ER pozit\u00edvneho karcin\u00f3mu prsn\u00edka<\/li>\n<li>Na predikciu odpovede pacienta na&nbsp;endokrinn\u00fa lie\u010dbu<\/li>\n<li>Na stanovenie heterogenity expresie estrog\u00e9nov\u00fdch receptorov pri prim\u00e1rnom n\u00e1dore a&nbsp;jeho metast\u00e1z.<\/li>\n<\/ul>\n<p>Indik\u00e1cia vy\u0161etrenia mus\u00ed by\u0165 schv\u00e1len\u00e1 multidisciplin\u00e1rnym t\u00edmom zameran\u00fdm na&nbsp;karcin\u00f3m prsn\u00edka v&nbsp;r\u00e1mci Komplexn\u00e9ho onkologick\u00e9ho centra.
