Wenn der Hämatologe Ihnen oder Ihrem nächsten Angehörigen eine Strahlentherapie empfohlen hat und Sie die Behandlungsmöglichkeiten besprechen möchten, wenden Sie sich an den Patientenservice des Protonenzentrums patient@ptc.cz.
Anschließend finden Sie die Indikationen für die evtl. Aufnahme einer RT in das Behandlungsschema:
- Befall im Mittelfellraum, wo das bestrahlte Volumen (die ursprüngliche Läsion) unter den Abstand der linken Herzkranzarterie oder unter den 7. Brustwirbel reicht,
- junge Frauen mit Befall in der Achselgrube, wo die Protonen-Radiotherapie die Brustdrüse schont und dadurch das Risiko für die Entwicklung eines strahlenbedingten Brusttumors minimiert,
- Patienten nach mehreren Chemotherapie-Reihen oder nach Knochenmarktransplantation, die ein erhöhtes Risiko für die Entwicklung von strahlenbedingten Komplikationen haben (vor allem Lungenbefall, Herz-Kreislauf-Befall und Knochenmarkbefall),
- aktive Erkrankung, eingeschränkt auf einen Bereich des Zwerchfells (supradiaphragmal oder subdiaphragmal), die auf die Chemotherapie oder biologische Behandlung nicht optimal angesprochen hat (vor allem PET positive Resterkrankung) und wo die Gesamtstrahlendosis erhöht werden muss,
- Patienten nach vorheriger Bestrahlung der gleichen oder anliegenden Region,
- Patienten mit einem bestehenden ernsthaften gesundheitlichen Problem (Zustand nach Herzinfarkt, Herzklappenfehler, Lungenbefall und weitere) und Bedarf an Bestrahlung dieser Region.

Offenes Seminar zur Indikation bei Lymphomen
Für Ärzte, die Interesse an einer Beratung bezüglich Indikation von Lymphom-Patienten haben, bietet das Kompetenzzentrum die Teilnahme an einem fachübergreifenden Seminar zur Indikation bei Lymphomen, das regelmäßig einmal die Woche im PTC stattfindet. Für die Teilnahme ist eine vorherige persönliche Anmeldung per E-Mail oder unter der angegebenen Telefonnummer erforderlich. Zu der Besprechung ist im Allgemeinen die relevante Bilddokumentation (kann per ePACS übermittelt werden) und die Epikrise vorzulegen.
Zur vorläufigen Orientierung siehe nachstehend die häufigsten Indikationen zur Protonen-Radiotherapie:
- Befall im Mittelfellraum, wo das bestrahlte Volumen (die ursprüngliche Läsion) unter den Abstand der linken Herzkranzarterie bzw. unter den Th7 reicht,
- junge Frauen mit axialem Befall, wo die Protonen-Radiotherapie die Brustdrüse schont und dadurch das Risiko für die Entwicklung eines sekundären Brusttumors minimiert,
- stark vorbehandelte Patienten nach mehreren Chemotherapie-Reihen oder nach Knochenmarktransplantation, die ein erhöhtes Risiko für die Entwicklung einer Radiotoxizität haben (vor allem Lungen-, Herz-Kreislauf-Toxizität und Knochenmarkbefall),
- Patienten mit lokalisiertem Befall, wo die Systemtherapie nicht angesprochen hat (vor allem PET opsitive mediastinale Residuen) und die Gesamtstrahlendosis erhöht werden muss,
- Patienten nach vorheriger Radiotherapie in der gleichen oder anliegenden Region,
Die Protonen-Radiotherapie ist kontraindiziert bei Patienten:
- mit Herzschrittmacher – absolute Kontraindikation,
- mit größeren Metallmengen am geplanten Bestrahlungsort.

Einen großen Anteil der zur RT indizierten Lymphom-Patienten bilden Patienten mit einer sehr guten Langzeitprognose. Diese Patienten sollten mit voller Kenntnis der Langzeitrisiken der Krebstherapie behandelt werden. Darunter gehören vor allem junge Patienten mit Hodgkin-Lymphom und einem prognostisch günstigen B-Non-Hodgkin-Lymphom (insbesondere mit den aggressiven Untertypen diffuses großzelliges B-Zell-Lymphom, primäres mediastinales B-Zell-Lymphom). Für diese Patienten sollte nicht der Ansatz der „Grenzdosen“ angewendet werden, sondern immer mindestens eine belastende RT-Technik erwogen werden. Den größten Nutzen hat die Protonen-Radiotherapie im Mittelfellraum. Aktuell wurde eine Empfehlung der International Lymphoma Radiation Oncology Group (ILROG) für die Protonen-Radiotherapie bei Erwachsenen mit Lymphomlokalisation im Mediastinum veröffentlicht, Verweis Dabaja BS(1), Hoppe BS(2), Plastaras JP(3). PROTON THERAPY FOR ADULTS WITH MEDIASTINAL LYMPHOMAS: THE INTERNATIONAL LYMPHOMA RADIATION ONCOLOGY GROUP (ILROG) GUIDELINES. Blood. 2018 Aug 14. pii: blood-2018-03-837633. Hier wurden Patienten identifiziert, die aus der Protonen-Radiotherapie den größten Nutzen ziehen. Es handelt sich vor allem um folgende Situationen:
- Bestrahlung im Mittelfellraum, wo das bestrahlte Volumen (die ursprüngliche Läsion) unter den Abstand der linken Herzkranzarterie reicht
- junge Frauen mit axialem Befall, wo die Protonen-Radiotherapie die Brustdrüse schont und dadurch das Risiko für die Entwicklung eines sekundären Brusttumors minimiert
- stark vorbehandelte Patienten, die ein erhöhtes Risiko für die Entwicklung einer Radiotoxizität haben (vor allem Lungen-, Herz-Kreislauf-Toxizität und Knochenmarkbefall).
Ein zahlenmäßig kleinerer Anteil entfällt auf Patienten mit refraktären oder relabierenden Lymhomen mit lokalisiertem Befall, meist einem umfangreichen Befund im Mediastinum, Verweis Rates of Toxicity and Outcomes After Mediastinal Proton Therapy For Relapsed/Refractory LymphomaTseng, Y.D. et al. Hier muss meist wegen der höheren Aggressivität der Erkrankung die Gesamtstrahlendosis erhöht werden und die standardmäßige Photonen-Radiotherapie birgt ein großes Risiko für strahlenbedingte Komplikationen. Die Protonen-Radiotherapie kann in diesem Fall als eigenständiges kuratives Verfahren oder als sog. „Debulking“ (Verkleinerung der Tumormasse) vor der weiteren geplanten Systembehandlung (zum Beispiel der allogenen Transplantation) in Anwendung kommen. So kann für einen Teil der Patienten die Protonen-Radiotherapie in Situationen, die früher als „inkurabel“ galten, ein Teil des kurativen Behandlungsverfahrens sein.
Mit ähnlicher Indikation kann die Protonen-Radiotherapie auch für wiederholte Bestrahlungen empfohlen werden, wo die Risikoorgane bereits durch die Strahlendosen der vorherigen Radiotherapie belastet sind. Auch hier lässt sich bei einem abgegrenzten Befall eine Langzeitkontrolle der Erkrankung erreichen.

Guidelines für die Protonen-Radiotherapie bei Lymphomen
Mit dem Thema Protonentherapie bei malignen Lymphomen beschäftigen sich einige inländische und internationale Leitlinien – Link (KLS, NCCN, ILROG Guidelines für mediastinale Lymphome.
Guidelines ILROG (International Lymphoma Radiation Oncology Group) für die Protonen-Radiotherapie bei mediastinalen Lymphomen bei Erwachsenen empfehlen dann strengere Regel für die Beurteilung der optimalen Bestrahlungstechnik bei Lymphom-Patienten; aus dieser Sicht ist dann der Einsatz der Protonen-Radiotherapie insbesondere bei 3 Patientengruppen zu erwägen. Es handelt sich um Patienten mit Mittelfell-Befall, der unter den Abstand der linken Herzkranzarterie reicht, junge Frauen, bei denen eine Bestrahlung im Bereich der Achselgrube erforderlich ist sowie Patienten nach mehreren Chemotherapie-Reihen oder anderer längerer Krebsbehandlung. In inländischen Quellen beschäftigt sich mit der Protonen-Radiotherapie (RT) in der Lymphom-Behandlung ein Teil des Kapitels zur Radiotherapie in „ Diagnostischen und therapeutischen Verfahren bei Patienten mit malignen Lymphomen“ der tschechischen Kooperativen Lymphomgruppe, aktualisierte 10. Ausgabe von 2018.
Eine Empfehlung der ASTRO (American Society for Radiation Oncology) klassifiziert die Tumore im Brustbereich (zu denen auch einige Lymphome gehören) als Tumore, bei denen die Kostenübernahme für die Radiotherapie erwogen werden sollte. Quellverweis.
Zu ausländischen Quellen, die die Protonen-Radiotherapie als alternative Bestrahlungstechnik mit einem potentiell schonenderen Umgang mit den umliegenden Risikoorganen empfehlen, gehören die NCCN Guidelines, aktuelle Version von 03/2018 (März 2018) (National Comprehensive Cancer Network – eine Allianz von weltweit führenden Krebszentren, die sich mit Patientenpflege, Forschung und Bildung beschäftigen).

Summary: Two techniques can improve the effectiveness of radiotherapy for locally advanced head and neck cancers, applying concomitant chemotherapy, known as concomitant boost (CB), and the use of alternative fractionation modes. For suitable patients, CB has shown to be feasible with acceptable toxicity and promising therapeutic outcomes.
